Pacific NW Surfacing (503) 381-3967 15 YEAR LIMITED WARRANTY-MAINTENANCE AGREEMENT FOR POLYCOAT SYSTEMS SAMPLE WARRANTY Pacific NW Surfacing warrants that the Surfacing System at _________________________________ __________________ ______ __________ with a total square footage of, ________, and completed on __________, will remain watertight, free from delamination, and free from excessive wear of deterioration for a period of FIVE years from the date of completion. Pacific NW Surfacing shall not be held liable for slope, damage, or delamination to our surface caused by failure of the existing structure or substrate. Slope and drainage is the responsibility of the owner unless Pacific NW Surfacing is specifically hired to slope the existing substrate. Warranty shall become effective upon payment of all charges for the project by its Owner, _________________________________. This Warranty is eligible for continuation, as described below: MAINTENANCE AGREEMENT Pacific NW Surfacing will provide an inspection of the surface every FIVE years, at no charge to the Owner. It is the Owner's responsibility to contact Pacific NW Surfacing for the warranty continuation inspection. This warranty continues for -2- FIVE year intervals, for a total of 15 years provided that within each FIVE year interval a new Elastomeric color coat is applied to any portion of the existing surface as deemed necessary by Pacific NW Surfacing. A renewal service charge of $2.50 per square foot, plus the current cost of materials, will be charged for each FIVE year renewal provided by Pacific NW Surfacing. This warranty terminates at the end of any FIVE year interval in which the Owner does not have Pacific NW Surfacing provide the inspection and renew necessary surfaces. Owner understands that physical damage and abuse is not covered and may void this warranty unless needed repairs, that would otherwise affect the waterproof integrity of the surface are immediately brought to the attention and repaired, at owners expense, by Pacific NW Surfacing. It is agreed by the Owner that Pacific NW Surfacing shall not be held liable for water damage to your property, our only liability is that we maintain the waterproof integrity of the surface. In the unlikely event that a leak does occur, we will make any necessary repairs in a timely and professional manner. The owner further understands that fading of the color surface over time is a normal occurrence. APPROVED: PACIFIC NW SURFACING

Pacific NW Surfacing shall not be held liable for slope, damage, or delamination to our surface caused by failure of the existing structure or substrate. MAINTENANCE AGREEMENT No maintenance, additional expense or follow up inspection is required. It is agreed by the Owner that Pacific NW Surfacing shall not be held liable for water damage to your property, our only liability is that we maintain the waterproof integrity of the surface. In the unlikely event that a leak does occur, we will make any necessary repairs in a timely and professional manner.
